Counter notice

- A statement under penalty of perjury that you have a good-faith belief the material was removed or disabled because of mistake or misidentification.
- A statement that you consent to the jurisdiction of the appropriate federal court and will accept service of process from the complaining party or that party's agent.
- Your physical or electronic signature.
